Pulmodyne’s O2-MAX System delivers continuous positive airway pressure (CPAP) throughout the breathing cycle. It provides CPAP at preset
levels throughout inspiration and exhalation, independent of the patient's ow rate. O2-MAX System is intended for use on spontaneously
breathing patients, there is no assembly required. SINGLE PATIENT USE ONLY.
INDICATIONS FOR USE:
To provide CPAP to spontaneously breathing adult (>30kg) patients in the hospital and pre-hospital (EMS) environment.
CONTRAINDICATIONS:
May be contraindicated for patients with any of these conditions:
• Facial lacerations
• Laryngeal trauma
• Recent tracheal or esophageal anastomosis
• Gastrointestinal bleeding or ileus
• Recent gastric surgery
• Basilar skull fracture
• Patients at high risk of vomiting
• Emphysematous Bulla - when an area of the lung may be brittle and present a risk of bursting
• Hypovolaemia - low blood volume
HOW IT WORKS:
O2-MAX Generator is a variable ow venturi device that uses an oxygen supply in conjunction with entrained air to generate an output ow.
O2-MAX Generator uses a 50psi (~4 bar) oxygen supply, and can generate ows up to 140 lpm and fractional inspired oxygen (FiO
2
) from
approximately 30% to 100%. The O2-CPAP valves, which are snapped onto the anti-asphyxia housing end of the circuit, are used to maintain
positive pressure ow rates from 60 to 140 lpm.
OPERATING THE SYSTEM:
• Connect directly to a 50psi (~4 bar) gas source
• Prior to use, be sure the device is free of obstructions and verify proper valve function
• The variable generator device is supplied in the ON position. To turn the unit to the OFF position, turn the knob fully clockwise.
• The variable generator device is supplied with the FLOW knob in the open position. To decrease the ow, turn the knob clockwise.
• The variable generator unit is supplied with the O
2
knob in the closed position. For higher oxygen concentrations, turn the O
2
knob
counterclockwise.
- The variable generator provides 30% O
2
ow when at the fully closed position as supplied.
- The variable generator provides 100% O
2
ow when at the fully open position after adjustment.
- The patient’s SpO
2
should be monitored using pulse oximetry at all times. When adjusting FiO
2
titrate oxygen until the
patient condition improves. Always follow local protocols. For exact measurements of FiO
2
user may consider the use of
an oxygen analyzer.
• Place mask over patients face. Utilize the head strap to secure the mask rmly in place
WARNING:
• Do not connect any gas supply other than oxygen to the O2-MAX System
• When not in use, disconnect O2-MAX oxygen line from 50psi (~4 bar) source.
• Watch the O2-CPAP valve to ensure that it remains open during inspiration
• Monitor with any in-line pressure gauge or manometer with a range of 0-30cm H
2
O per local established protocol. If the pressure drops
signicantly during patient inspiration then the ow is too low, increase the ow rate to the patient.
• For use only by thoroughly trained personnel
• Do not use O2-CPAP valve if it becomes occluded. An occluded O2-CPAP valve may obstruct patient’s exhalation and result in potential
injury. If this occurs, discard entire system or remove the occluded O2-CPAP valve from the system and replace with another O2-CPAP
valve.
• The circuit has an anti-asphyxia valve which prevents exhalation into the tubing in the event of no fresh gas ow. If the fresh gas ow fails,
the anti-asphyxia valve in the circuit will minimize the risk of asphyxia when there is no oxygen pressure. When there is not oxygen ow
from the generator, the mask should not be worn.
MONITORING THE PATIENT:
During operation, be sure to check the following on a regular basis:
• Ensure that there are no leaks at the patient connection
• Ensure that there is ow from the O2-CPAP valve during inspiration (which means that the generator is supplying adequate ow to meet
patient demand). Optionally, monitor the in-line pressure gauge or manometer during inspiration. If the pressure drops then the ow is
inadequate
• Monitor the patient’s arterial blood oxygen saturation (SaO
2
)
• Monitor the patient’s SpO2 continuously using pulse oximetry
• Monitor the patient for signs of dehydration and discomfort in the upper airways

INDIKATIONEN:
Bereitstellung von CPAP für spontan atmende erwachsene (>30 kg) Patienten im Krankenhaus und in präklinischer Umgebung (Rettungsdienst).
KONTRAINDIKATIONEN:
Kann bei Patienten mit folgenden Erkrankungen kontraindiziert sein:
• Gesichtsverletzungen
• Kehlkopftrauma
• Kürzliche tracheale oder ösophageale Anastomose
• Gastrointestinale Blutungen oder Ileus
• Kürzliche Magenoperation
• Schädelbasisbruch
• Patienten mit hohem Risiko von Erbrechen
• Emphysemblase – ein Bereich der Lunge ist brüchig und es besteht das Risiko, dass Lungenbläschen platzen
• Hypovolämie – niedriges Blutvolumen
FUNKTIONSWEISE:
Der O2-MAX Generator ist ein Gerät mit Venturi-Düse und variablem Durchuss, das mithilfe der Sauerstoffzufuhr und der eingeschlossenen Luft einen
Ausgangsuss erzeugt. Der O2-MAX Generator verwendet eine Sauerstoffzufuhr von 50 psi (~4 bar) und kann Durchussraten von bis zu 140 l/min sowie eine
inspiratorische Sauerstoffkonzentration (FiO
2
) von 30 % bis 100 % erzeugen. Die O2-CPAP-Ventile, die am Ende des Schlauchs mit dem Anti-Asphyxie-Gehäuse
angebracht sind, dienen der Aufrechterhaltung eines Durchusses mit positivem Druck von 60 bis 140 l/min.
BEDIENUNG DES SYSTEMS:
• Schließen Sie das Gerät direkt an eine 50 psi (~4 bar) Gasquelle an.
• Versichern Sie sich vor der Verwendung, dass das Gerät nicht verstopft ist und prüfen Sie das Ventil auf ordnungsgemäße Funktion.
• Der variable Generator ist bei der Auslieferung auf die Position EIN voreingestellt. Zum AUSSCHALTEN drehen Sie den Knopf bis zum Anschlag im
Uhrzeigersinn.
• Der variable Generator wird mit dem FLOW-Knopf (Durchuss) in der offenen Position geliefert. Drehen Sie den Knopf im Uhrzeigersinn, um den Durchuss zu
reduzieren.
• Der variable Generator wird mit dem O
2
-Knopf in der geschlossenen Position geliefert. Drehen Sie den O
2
-Knopf gegen den Uhrzeigersinn, um die
Sauerstoffkonzentration zu erhöhen.
- Der variable Generator liefert einen O
2
-Fluss von 30 %, wenn er wie bei der Auslieferung auf die vollständig geschlossene Position eingestellt ist.
- Der variable Generator liefert einen O
2
-Fluss von 100 %, wenn er nach der Anpassung vollständig geöffnet ist.
- Die partielle Sauerstoffsättigung (SpO
2
) des Patienten sollte jederzeit mit einem Pulsoximeter überwacht werden. Wenn Sie die FiO
2
einstellen, titrieren Sie den
Sauerstoff, bis sich der Patientenzustand verbessert. Befolgen Sie immer die örtlichen Krankenhausprotokolle. Die Verwendung eines Sauerstoffanalysators
sollte in Erwägung gezogen werden, um exakte Messungen der FiO
2
zu erhalten.
• Legen Sie die Maske auf das Gesicht des Patienten. Verwenden Sie das Kopfband, um die Maske sicher zu befestigen.
WARNHINWEIS:
• Verbinden Sie keine andere Gasquelle als Sauerstoff mit dem O2-MAX System.
• Trennen Sie die O2-MAX Sauerstofeitung von der 50 psi (~4 bar) Quelle, wenn das Gerät nicht in Gebrauch ist.
• Beobachten Sie das O2-CPAP-Ventil, um sicherzustellen, dass es beim Einatmen offen bleibt.
• Überwachen Sie die Beatmung mit einem integrierten Druckmesser oder Manometer mit einem Messbereich von 0-30 cm H
2
O gemäß Krankenhausprotokoll.
Falls der Druck während der Einatmung des Patienten deutlich fällt, ist der Flow zu niedrig eingestellt. Erhöhen Sie die Durchussrate zum Patienten.
• Nur von hierfür intensiv geschultem Personal zu verwenden.
• Verwenden Sie das O2-CPAP-Ventil nicht, wenn es verstopft ist. Ein verstopftes O2-CPAP-Ventil kann die Ausatmung des Patienten behindern und zu möglichen
Verletzungen führen. In diesem Fall schalten Sie das gesamte System aus oder entfernen Sie das verstopfte O2-CPAP-Ventil vom System und ersetzten Sie es
durch ein anderes O2-CPAP-Ventil.
• Das Schlauchsystem ist mit einem Anti-Asphyxie-Ventil versehen, das das Ausatmen in das Schlauchsystem verhindert, wenn kein frischer Sauerstoff zugeführt
wird. Bei einem Ausfall der Sauerstoffzufuhr minimiert das Anti-Asphyxie-Ventil im Schlauchsystem das Risiko einer Asphyxie, wenn kein Sauerstoffdruck
vorhanden ist. Wenn die Sauerstoffzufuhr vom Generator unterbrochen ist, sollte die Maske nicht getragen werden.
ÜBERWACHUNG DES PATIENTEN:
Während des Betriebs müssen die folgenden Faktoren regelmäßig überprüft werden:
• Achten Sie darauf, dass am Patientenanschluss keine Leckagen auftreten.
• Stellen Sie sicher, dass beim Einatmen ein Durchuss durch das O2-CPAP-Ventil erfolgt (d. h. der Generator liefert einen ausreichenden Durchuss für die
Atemanforderungen des Patienten). Als Alternative hierzu können Sie den integrierten Druckmesser oder das Manometer während des Einatmens beobachten.
Wenn der Druck fällt, ist der Durchuss unzureichend.
• Überwachen Sie die arterielle Sauerstoffsättigung im Blut des Patienten (SaO
2
).
• Überwachen Sie die SpO2 des Patienten kontinuierlich mit dem Pulsoximeter.
• Überwachen Sie den Patienten auf Anzeichen von Dehydratation und Beschwerden der oberen Atemwege.
