Painful and disabled knee joint from arthritis or deformity, revision of failed procedures.
Absolute contraindications include infection; relative include uncooperative patient, osteoporosis, etc.
Assess bone stock, ligament instability, measure valgus angle, estimate femoral size.
Instrumentation designed for standard technique; release patella tendon for space, evaluate scarring.
Tilt patella, remove tissues, determine thickness and resection using caliper and guide.
Mill patella, determine thickness, size using mill bushings and appropriate tools.
Use single peg drill guide to locate central peg, drill hole with patellar drill.
Use 3-peg drill guide, drill holes for component pegs with patellar drill.
Attach reamer, mill bushing, magnetic spacer to determine depth and ream patella.
Prepare inset surface, establish resection depth, drill holes for pegs.
Set valgus angle dial and resection level dial on the adjustable distal cut guide.
Assemble IM rod and adjustable distal cut guide, introduce rod into femoral canal.
Attach distal cut block to adaptor, then to cut guide, pin block in place.
Assemble valgus wing onto IM rod, slide until flush, attach block, pin in place.
Remove valgus wing, use saw blade to complete distal resection through the selected slot.
Flex knee >90 deg to avoid ACL; resect 2-3mm to respect joint line with intact ACL.
Use adjustable or fixed A/P sizer feet to assess femoral axis and optimize rotation.
Hyperflex knee, place sizer flush, drill location holes, read component size from central scale.
Choose 4-in-1 block matching size, assemble ACL protector, place into distal femur holes.
Resect anterior/posterior bone and chamfers with saw blade using the block.
Use narrow saw blade for posterior cuts; perform notchplasty for ACL clearance.
Select tibial island template to size and set rotation, outline island with marking pen.
Attach tibial universal cutting block to EM tibial guide, position guide on tibia with ankle clamp.
Place knee in max flexion, use Z-retractor, select tibial template for A/P and M/L coverage.
Select tibial trial and Vanguard XP-XP bearing trials based on tibial template size.
Free float trial, place femoral trial, check range-of-motion and stability.
Select appropriate medial/lateral Vanguard XP-XP bearing trials based on tibial trial size.
With trial components in place, check range-of-motion and stability of the knee.
Select and insert Vanguard XP tibial bearings onto tibial tray; push posteriorly with finger pressure.
