RETURN MERCHANDISE AUTHORIZATION
WARNING
Before products are returned to Bray for repair or service
Bray must be provided with a certificate that confirms that the
product has been decontaminated and is clean
All products that are returned require a Return Merchandise
Authorization (RMA) Contact a Bray representative to obtain
authorization and shipping instructions
The following information must be provided when submitting RMA
Serial number
Part number
Month and year of manufacture
Time of purchase (if known)
Actuator and actuator accessoriescontrols specifics
Application
Media
Operating temperature
Operating pressure
Total estimated cycles (since last installation or repair)
NOTE Product information is provided on identification tag
attached to device
NOTICE
Materials must be cleaned and sanitized prior to return MSDS
sheets and Declaration of Decontamination are required
