• WARNINGS – General:
1. Failure to comply with all directions and warning may result in injury or death;
use only as directed.
2. This product is not suitable for all individuals. Other devices may be required.
3. Inspect the product for damage before each use and do not use it if it appears to
be damaged or not functioning properly.
4. Never alter the product in any way.
5. This product is designed for indoor use only within close proximity to skilled
caregivers.
• WARNING – This mattress extender is designed to extend an existing
complimentary mattress to provide a comfortable sleeping surface and may
require other equipment for, among other things, entrapment and elopement
protection.
• WARNING – The mattress extender is designed to be used only with foam
mattresses of a similar composition. Do not use the mattress extender with an air
or inflatable mattress.
• WARNING – Do not use the mattress extender with a mattress of a differing
height or width. Prior to each use, check to ensure that the mattress and mattress
extender combination does not pose an entrapment risk to the resident.
• WARNING – The mattress extender is designed to be used to reduce unsafe
gaps in the support surface and not as a substitute for the resident’s sleep surface.
Be sure to check that the resident’s pressure reducing needs are being properly
addressed by the sleep surface in conjunction with the use of this product.
