Overview

The Ottobock 50R59 Dorso Direxa Posture is a thoracolumbar brace designed for orthotic fittings of the back, intended exclusively for contact with intact skin. It supports active straightening of the thoracic spine and activates its muscular stabilization, promoting sensorimotor function, alleviating pain, and relieving the thoracic spine through active straightening and muscular stabilization. It also supports the lumbar spine by increasing intra-abdominal pressure and providing external stabilization.

Indications for Use:

The brace is indicated for:

  • Poor body posture (hunchback without fused vertebrae, Scheuermann's disease).
  • Osteoporosis of the thoracic and lumbar spine.
  • Aftercare of osteoporotic fractures of the thoracic and/or lumbar spine.
  • Thoracic and lumbar spine syndrome, dorsolumbalgia, dorsolumboischialgia, muscular imbalances in the thoracic and lumbar spine, and ligament sprains of the thoracic and lumbar spine.
  • Slight degenerative changes in the thoracic and lumbar spine (e.g., osteochondrosis, facet joint syndrome). The indication must be determined by a physician.

Contraindications:

  • Absolute Contraindications: None known.
  • Relative Contraindications: Consultation with a physician is required for skin diseases/injuries, inflammation, raised scars with swelling, reddening, and warmth in the area where the support is fitted, tumors, lymphatic congestion (including unclear soft tissue swelling distal to the support), and dysesthesia in the trunk.

  • Waist Circumference (approx. 2 cm/0.8 inch above the iliac crest):
    • XS: 70–80 cm (27.6–31.5 inches)
    • S: 80–90 cm (31.5–35.4 inches)
    • M: 90–100 cm (35.4–39.4 inches)
    • L: 100–110 cm (39.4–43.3 inches)
    • XL: 110–120 cm (43.3–47.2 inches)
    • XXL: 120–130 cm (47.2–51.2 inches)
  • Washing Temperature: 40 °C (104 °F)

Usage Features:

  • Sizing: Measure the waist circumference approximately 2 cm (0.8 inch) above the iliac crest to determine the correct brace size using the provided size chart.
  • Fitting the Metal Rods: The two metal rods can be removed from their pockets and individually fitted to the spine, then reinserted.
  • Optional Pad Installation: A pad can be attached to the inside of the brace using a hook-and-loop closure, ensuring it is at least 1 cm (0.4 inch) above the seat surface when seated.
  • Application:
    1. Place the upper part of the brace over the shoulders like a backpack.
    2. Wrap the lower part of the brace around the trunk.
    3. Insert fingers into the hand straps of the abdominal closure.
    4. Pull the brace forward with equal force on both sides.
    5. Attach the right part of the abdominal closure over the left part.
    6. Remove fingers from the left hand strap first, then the right. Ensure hook-and-loop sections lie flat and do not contact clothing or skin.
    7. Route the ends of the shoulder strap forward around the chest and secure the hook-and-loop closure below the chest.
    8. To increase compression, pull the ends of the elastic straps forward and attach them to the bottom closure.
  • Final Check: After fitting, verify the brace's correct fit and positioning. The brace is correctly seated when its lower edge extends just below the groin.
  • Patient Instruction: Patients should be instructed on how to properly don and doff the brace and ensure its correct fit. Practice these steps with the patient.
  • Important Safety Notes:
    • Do not wear the product directly on the skin to avoid irritation from chafing or sweat.
    • Ensure the product is applied properly and fits correctly to prevent local pressure points and constriction of blood vessels and nerves.
    • Do not expose the product to open flames, embers, or other heat sources.
    • Avoid contact with oils, salves, lotions, or other products containing oils or acids, as this can lead to insufficient stabilization due to material degradation.
    • The product is for single-person use only.
    • Before each use, check the product for functional reliability, wear, and damage. Discontinue use if the product is no longer functional, worn, or damaged.
    • The daily duration of use and period of application are typically determined by a physician.
    • Initial fitting and application must be performed by qualified personnel.
    • Patients should seek immediate medical attention if they notice any unusual changes (e.g., worsening of symptoms).

Maintenance Features:

  • Cleaning:
    1. Close all hook-and-loop closures.
    2. It is recommended to use a laundry bag or net.
    3. Wash the brace in warm water at 40 °C (104 °F) with a standard mild detergent. Do not use fabric softener. Rinse thoroughly.
    4. Allow to air dry. Do not expose to direct heat sources (e.g., sunlight, stove, or radiator).
    5. Only clean the product with approved cleaning agents to prevent damage.
