HDPlex H5 - Apply Thermal Pad and Heat Sinks

A highly conductive thermal pad is included in the H5 package, which should be
applied to the MOSFET and inductors on the GPU board.
Install heat sink blocks (Not included) on the thermal pad to increase heat
dissipation from the MOSFET and inductors.
