Should you require repair or replacement during this initial 90 day warranty period, please contact truMedic
at 888 264 1766 or email service@truMedic.com for return information. Please retain your sales receipt, as
it will be required to receive service under this warrantee. Out-of-warrantee service is provided on a
case-by-case basis. A fee for repair or replacement may apply on out of warrantee work.

Do not use this device if you have an implanted defibrillator or implanted metallic devices. Such use could
cause electrical shock, burns, electrical interference or death.
Simultaneous connection of a patient to a h.f. surgical equipment may result in burns at the site of the
stimulator electrodes and possible damage to the stimulator.
