difficulty
experienced
(including
all
pertinent
information
such
as
flow
rate,
administration
set
used

the
message
displayed
at
the
time
of
the
difficulty.
Important:
Confirm
pump
failure
as
described
in
this
manual
before
return.
Unconfirmed
complaints
are
required
to
go
through
Production
Testing
and
Quality
Assurance
Inspection
and
will
be billed
accordingly
(see
warranty).

(RA)
number.

A
purchase
order
number
or
other
authorized
form
of
payment
will
be
required
on
non-warranty
repair
before
a
RA
number
is
issued.

RA

applicable

Purchase

Written

your

both
the
independent

cases
where
SIGMA
is

dealing

Package
pump
in
the
original
shipping
container
(replacement
available
upon
request) and
affix
the
RA
number
on
the
outside
of
the
package.

Ship
pump,
freight
prepaid,
by
traceable
means

SIGMA
International,
LLC.

When
a
device
is
warranted
or
under
service
agreement,
it
is
returned
freight
prepaid.
Field
Service
.
Authorization:
Verbal
authorization
must
be
obtained
in
advance
before
field
service
can
be
performed.
Written
documentation
will also
be
required.
Parts:
While
under
warranty,
SIGMA
will
supply
factory
trained/authorized
service
technicians
with
no
charge
parts
as
needed.
All
defective
warranty
parts
must
be
returned
to
SIGMA.
