Including Observations and Assessments in a Patient Record
The Observations and Assessments Menu
Touching the Observations and Assessments or Pain Index pane opens the
Observations and Assessments menu. You can open the menu only if your system
administrator has enabled QuickCapture. If QuickCapture is not enabled, the
Observations and Assessments pane displays the word Disabled.
The Observations and Assessments menu includes the following features:
• In all configurations:
- A measurements bar shows the patient’s current vital signs measurements.
- A Free Text button enables you to open a Free Text window into which you can
enter up to 100 characters. The text you enter is saved to the patient record. It
can be viewed by touching the Free Text button in the expanded patient record.

• When the default QuickCapture configuration is enabled, the Observations and
Assessments menu provides a pain scale, from which you can select a value from 0
to 10, or touch one of six faces to indicate a pain value of 0, 2, 4, 6, 8, or 10. The pain
level you select is displayed in the Pain Index pane on the main screen and is
included in the patient record.
• When a custom QuickCapture configuration is enabled, the default pain scale is
replaced by one or more tabs containing customized fields to be completed. Within
each tab, required fields are indicated by a yellow asterisk. Depending on the
QuickCapture configuration, you may be prevented from entering invalid data, such
as the wrong unit of measure for a measurement.
Entering Observations and Assessments
WARNING
• To ensure patient safety, always double check that the QuickCapture observations and
assessments you have entered are correct before saving the patient record.
• QuickCapture and QuickAlerts configurations can vary among the different departments in your
facility. Using the wrong configurations can result in patient injury. Before using a monitor that
has been moved from another department, confirm with your administrator that the monitor has
the appropriate QuickCapture and QuickAlerts configurations for your department.
When you manually save a record (by touching the Save or Save Now button on the
main screen), observations and assessments are saved to the patient record and are
included when the record is exported to the EMR system.
Observations and assessments are not automatically saved with other measurements in
Interval or Continuous monitoring mode. To save observations and assessments in these
modes, you must touch Save Now on the main screen.
