The motor may be overloaded, the control gear could be defective, the starting torque might be too high, the rotor could be defective, there could be a short circuit to earth, or there might be a fault in the starter or star/delta switch. You should check the voltage on each phase, reduce the load, examine the control gear for bad contacts, change to a higher tap if using autotransformer starting, look for broken rings, check with a Megger, and inspect the contacts and connections of the starter or switch.
