LIMITED WARRANTY
3-YEAR LIMITED WARRANTY ON NEW INSTRUMENTS: Instrumentation
purchased new from Welch Allyn, Inc. (Welch Allyn) is warranted to be free from original
defects in material and workmanship under normal use and service for a period of three years
from the date of first shipment from Welch Allyn. This warranty shall be fulfilled by Welch
Allyn or its authorized representative repairing or replacing at Welch Allyn's discretion, any
such defect, free of charge for parts and labor.
Welch Allyn should be notified via telephone of any defective product and the item should
be immediately returned, securely packaged and postage prepaid to Welch Allyn. Loss or
damage in shipment shall be at purchaser's risk.
Welch Allyn will not be responsible for loss associated with the use of any Welch Allyn
product that (1) has had the serial number defaced, (2) has been repaired by anyone other than
an authorized Welch Allyn Service Representative, (3) has been altered, or (4) has been used
in a manner other than in accordance with instructions.
1-YEAR LIMITED WARRANTY ON PROBES AND PROBE COVERS:
Welch Allyn warrants probe covers and probes to meet Welch Allyns specifications for the
Product at the time of purchase and to be free from original defects in material and
workmanship under normal use and service for a period equal to 1 year from the date of first
shipment of such Product to the customer by or on behalf of Distributor.
THIS WARRANTY IS EXCLUSIVE AND IN LIEU OF ANY IMPLIED WARRANTY OF
MERCHANTABILITY, FITNESS FOR PARTICULAR PURPOSE, OR OTHER WAR-
RANTY OF QUALITY, WHETHER EXPRESSED OR IMPLIED. WELCH ALLYN
WILL NOT BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES.

Overview

The Welch Allyn SureTemp Model 678 is a portable thermistor thermometer designed for measuring patient temperatures. It offers both Normal and Monitor operating modes for various temperature-taking scenarios. The device is intended for use with single-use disposable probe covers to minimize cross-contamination.

Function Description

The primary function of the SureTemp Model 678 is to accurately measure patient temperatures within a range of 84.0°F to 108.0°F (28.9°C to 42.2°C). It can take oral, axillary, and rectal temperatures. In Normal mode, the thermometer's microprocessor "predicts" body temperature, providing quick readings: approximately 4 seconds for oral temperatures and 15 seconds for rectal temperatures. For axillary temperatures in patients under 4 years, Normal mode displays readings in about 10 seconds. The Monitor mode continuously displays the probe's temperature, reflecting the patient's actual oral or rectal temperature in about 3 minutes, and for axillary temperatures in patients 4 years and older, in about 5 minutes. This mode is suitable for long-term temperature monitoring or when accurate readings are difficult to obtain in Normal mode.

The device is equipped with a Mode Button that serves multiple purposes: recalling the last Normal mode temperature, converting temperatures between °F and °C, switching the unit to Monitor Mode, and allowing the clinician to change the measurement site (Oral to Axillary or vice versa). A Pulse Timer Button is also included, used to time pulse, respiration, IV drip rate, or other timed measurements. It provides audible beeps at 0, 15, 30, 45, and 60 seconds, automatically turning off after a full 60 seconds. The timer can be stopped by pressing the Pulse Timer or Mode button, or by removing the probe from its storage channel.

The thermometer features a display that shows various indicators:

  • Instrument Malfunction Icon: Indicates if the unit is not functioning properly, requiring contact with Welch Allyn Customer Service.
  • Broken Probe Icon: If displayed, the probe may be damaged. Users should remove and reinstall the probe; if the error persists, a new probe should be installed.
  • Probe Position Icon: Indicates that the probe has lost contact with the tissue. Once proper contact is re-established, the icon will turn off but will reappear at the end of the measurement. A new temperature measurement is recommended.
  • ALy or A (Axillary Probe) Icon: Shows that the axillary mode has been selected.
  • OrL or O (Oral Probe) Icon: Shows that the oral mode has been selected.
  • rEC or R (Rectal Probe) Icon: Shows that the rectal mode has been selected.
  • Walking Segments: These segments are displayed when the thermometer is in the process of taking a temperature in Normal mode.
  • M (Monitor Mode) Icon: Indicates that Monitor mode is active, generally used for long-term temperature monitoring, taking adult axillary temperatures, or when difficult situations prevent accurate temperatures in Normal mode.
  • P (Pulse Timer) Icon: Displayed when the pulse timer is in use.
  • High Temperature Icon: Displayed if the patient's temperature is greater than 108.0°F (42.2°C).
  • Low Temperature Icon: Displayed if the patient's temperature is less than 84.0°F (28.9°C).
  • High Ambient Temperature Icon: Displayed if the ambient temperature is greater than 104°F (40°C).
  • Low Ambient Temperature Icon: Displayed if the ambient temperature is less than 60.8°F (16.0°C).
  • Low Battery Icon: Flashes when batteries are low and remains on when batteries are dead.
  • SEC (Security Mode) Icon: Indicates that the thermometer is in security mode and must be returned to the wall holder to resume normal operation.
  • Display Test: All segments of the thermometer display appear briefly when the probe is removed from the storage channel to ensure proper function.

Usage Features

The SureTemp Model 678 is designed for ease of use with specific procedures for different temperature measurements:

Setup:

  1. Install the appropriate Oral/Axillary or optional Rectal probe by inserting the latching plug into the receptacle on the unit. The plug snaps into place and requires depressing the latch for removal.
  2. Observe the LCD to confirm the displayed probe type (OrL, ALy, or rEC).
  3. Insert the probe shaft completely into the probe storage channel.
  4. Remove the probe from the storage channel.
  5. Observe the LCD to confirm all display segments appear, followed by the probe type and temperature scale (°C or °F). Replace the probe in the storage channel.
  6. Insert a box of disposable probe covers (part number 05031) into the probe cover storage well.

Changing Probes:

To change probes, depress the latch and unplug the probe connector, remove the probe from the storage channel, and then repeat the setup steps.

Taking Oral Temperatures (Blue Capped Probe):

  1. Withdraw the probe from the storage channel; a display test and audible tone will occur, followed by the probe type display (OrL).
  2. Load a probe cover onto the probe by holding the probe handle collar with thumb and forefinger, being careful not to press the ejection button, and firmly pressing the collar down to secure the cover.
  3. With OrL displayed, gently insert the probe tip into the patient's slightly opened mouth, sliding it under the tongue into the sublingual pocket. The patient should close their lips around the probe.
  4. Hold the probe in place, ensuring the tip remains in contact with tissue throughout the measurement for fast and accurate results.
  5. During measurement, "walking" segments will appear on the display.
  6. When the final temperature is reached, an audible tone will sound, and the temperature will be displayed.
  7. After measurement, remove the probe, eject the cover by pressing the ejection button, and insert the probe into the storage channel to clear the display for the next measurement.

Taking Axillary Temperatures (Blue Capped Probe):

  • Patients Under 4 Years:
    1. Withdraw the oral probe and observe the self-test.
    2. With OrL displayed, press and hold the Mode button for two seconds until ALy appears, indicating Axillary mode. (Note: The thermometer can be set to default to Axillary mode in Biotech mode).
    3. Attach a probe cover.
    4. Lift the patient's arm to visualize the axilla.
    5. Place the probe as high as possible in the axilla, ensuring the tip is completely surrounded by axillary tissue. Avoid premature tissue contact to prevent inaccurate readings.
    6. Hold the arm snugly at the patient's side without movement during measurement.
    7. An audible tone and temperature display indicate the final temperature.
    8. Remove the probe, eject the cover, and insert the probe into the storage channel to clear the display.
  • Patients 4 Years Through Adult:
    1. Withdraw the oral probe and observe the self-test.
    2. Attach a probe cover.
    3. Place the probe in the axilla and place the arm snugly at the patient's side.
    4. After the temperature is displayed, depress the Mode button for more than 2 seconds until the "M" icon appears, indicating Monitor mode. Continue taking the temperature for about 5 minutes.
    5. After measurement, insert the probe into the storage channel to clear the display and reset to Normal mode.

Taking Rectal Temperatures (Red Capped Probe):

  1. Exchange the oral probe for the rectal probe by following the SETUP steps.
  2. Attach a probe cover, ensuring rEC is displayed.
  3. Separate the buttocks and gently insert the probe only 1.5 cm (5/8 inch) into the rectal sphincter (less for infants and children). Lubricant is optional.
    • WARNING: Incorrect insertion can cause bowel perforation.
  4. Tilt the probe so the tip touches tissue and hold it in place throughout the measurement.
  5. When the final temperature is reached (about 15 seconds), a tone will sound, and the temperature will be displayed.
  6. Remove the probe, eject the cover, and insert the probe into the storage channel to clear the display.
  7. Wash hands to reduce cross-contamination.

Monitor Mode Usage:

To enter Monitor mode, remove the probe, take a temperature, and after it's displayed, depress the Mode button for more than 2 seconds until the "M" icon appears. The unit will continuously display temperatures. Replacing the probe in the storage channel clears the display and resets the thermometer to Normal mode. The unit can also automatically enter Monitor mode by removing the probe and waiting 60 seconds. For accurate readings in Monitor mode, allow 3 minutes for oral and rectal temperatures, and 5 minutes for axillary temperatures, to stabilize.

Security Features:

The SureTemp Model 678 includes two security alarm systems: an Anti-Theft Temperature Counter System and an Instant Anti-Theft Audio Alarm System. When triggered, "SEC" is displayed, audio tones sound, and the unit becomes inoperable. To reset, the unit must be returned to its wall holder.

Entering Biotech Mode (for Security Setup):

  1. Place the unit into the wall holder.
  2. Press and hold the Mode button while removing the probe from the probe holder.
  3. Remove the thermometer from the wall holder. Replacing the probe into the holder will return the thermometer to normal operating state.

Setting Security Features in Biotech Mode:

  • To navigate Biotech functions, press and hold the Mode button for approximately 2 seconds until the function number changes and a double beep is heard. Release the Mode button.
  • Once the desired function number is reached, press and immediately release the Mode button to toggle settings.
  • Function 5: Temperature Count Security Feature: This sets the number of temperatures that can be taken (0, 25, 50, 100, 200) before the unit enters security alarm mode and must be returned to the wall holder. Setting to "0" disables this function.
  • Function 6: Instant-On Security Feature: When set to ON, the thermometer immediately goes into security alarm when removed from the wall holder. Setting to OFF disables this alarm.
  • After setting, replace the probe into the holder to exit Biotech mode.

Wall Mounting Instructions:

The wall mount bracket should be mounted approximately 48 inches from the floor, with at least 12 inches of clearance above and 4 inches from the bottom and sides. This ensures easy access to the probe and probe covers, prevents hindrance, and allows the display to be read from the patient's bedside without stressing the instrument, wall mount, or patient.

Maintenance Features

Beyond using a new probe cover for each reading, keeping the instrument clean, and replacing batteries, no other user maintenance is required. Detailed information on security settings, sterilization, and calibration verification is available in the 678 SureTemp Technical Service Manual.

Battery Replacement:

  1. Firmly press in on the indentations on the sides of the rear housing to release two snaps.
  2. Remove the battery door.
  3. Lift the battery strap to remove the old batteries.
  4. IMPORTANT: After removing old batteries, press and hold the MODE button for 5 seconds before inserting new ones.
  5. Replace with 3 AA Alkaline batteries, orienting them according to the markings inside the battery well.
  6. Replace the battery door by inserting two tabs into the bottom of the thermometer and snapping it back into place.

Cleaning & Sterilization:

The 678 SureTemp housings can be cleaned with a cloth dampened with alcohol, warm water, or a nonstaining disinfectant such as Sporicidin® Spray and Towelettes or MetriSpray™. Care should be taken not to scratch the LCD faceplate. Low temperature Ethylene Oxide (ETO) sterilization of Oral or Rectal probes should only be used when absolutely necessary. The procedure for ETO sterilization is described in the 678 SureTemp Technical Service Manual.

Safety & Warnings:

  • Do not use in the presence of flammable anesthetics.
  • Federal (USA) law restricts this device to sale by or on the order of a physician.
  • Read the booklet thoroughly for accurate results and patient safety. Contact Customer Service for technical or clinical questions.
  • Use only single-use disposable Welch Allyn probe covers to limit cross-contamination. Using other covers or failing to use one may cause temperature errors and invalidate the warranty.
  • Oral probes (blue ejection button) are for oral and axillary temperatures only. Rectal probes (red ejection button) are for rectal temperatures only. Using the wrong probe will cause temperature errors.
  • The thermometer case is not waterproof; do not drip fluids onto it.
  • Do not use this instrument for any purpose other than specified in this booklet; doing so will invalidate the warranty.
  • Do not use the thermometer if there are signs of damage to the probe or instrument.
  • Do not ETO sterilize without using proper procedures described in the 678 SureTemp technical manual.
