B. Dispose of through the manufacturer,
so:
the stroller must be sent to the manufacturer at the user's expense.
In this case, the shipment must be accompanied by a statement of the stroller user
with the following data:
• with a declaration on the transfer of the stroller for liquidation,
• with information about the serial number of the stroller (embossed on the right chain of
the backrest inclination adjustment mechanism and given in the WARRANTY CARD),
with the date and signature of the handing over the stroller.
Remark:
in the absence of the above statement, the shipment will be returned to the sender at his
expense,
the cost of disposal carried out by the manufacturer is PLN 240 (gross), (PLN 195,- net).
VII. WHAT TO DO IN THE EVENT OF A SERIOUS INCIDENT RELATED TO A DEVICE
(STROLLER)
In the event of a serious incident* related to the COMFORT stroller, the information should be
reported to the Manufacturer and to the Office for Registration of Medicinal Products, Medical
Devices and Biocidal Products.
* a serious incident means that it has directly or indirectly led, could have led or may have led to:
• death of a patient, user or other person
• temporary or permanent serious deterioration of the health of a patient, user or other
person
• serious threat to public health
