Select Telecom: - Others
Select 3G Network:- Let it be Default.
Select Username: - Refer the Table.
Select the password:- Refer the Table.
Dial Number:- Refer the Table.
Select Authentication:- Auto.
APN:- Refer the table.
Press NEXT.
