Overview

The Guardian Fall Protection Bolt Hole Anchor (Part #: 00230) is a Type A anchorage connector designed for personal fall arrest, restraint, work positioning, and rescue/confined space applications. It is intended for use in steel substrates only, with a minimum thickness of 1/4 inch and a compatible installation hole diameter ranging from 3/4 inch (19mm) to 1 3/8 inches (35mm). The anchor must be installed at a minimum distance of 2 inches from all edges or corners.

Function Description:

The Bolt Hole Anchor serves as a secure connection point for personal fall arrest systems (PFAS) and other fall protection equipment. It is designed to be inserted into a pre-drilled hole in a steel substrate, where a spring-loaded blade expands to secure the anchor in place. The anchor provides a connection point for a single PFAS, supporting a maximum of one user. It is rated for loading in any direction, provided it is installed as prescribed, and is suitable for use in Horizontal Lifeline (HLL) applications.

Important Technical Specifications:

  • Compliance: Meets or exceeds OSHA 1926 Subpart M, OSHA 1910, ANSI Z359.18-2017, and ANSI A10.32-2012 standards for fall protection. Also tested in compliance with ANSI/ASSE Z359.7.
  • Materials: Constructed from steel, stainless steel, and aluminum.
  • Minimum Breaking Strength (MBS): 5,000 lbs.
  • Worker Weight Capacity Range: 130-420 lbs. (including all clothing, tools, and equipment).
  • Minimum Service Temperature: -30° F.
  • Maximum Connections: 1 attachment per connection point.
  • Substrate Requirements:
    • Material: Steel only.
    • Minimum Thickness: 1/4 inch.
    • Installation Distance: Minimum 2 inches from all edges or corners.
    • Hole Diameter: 3/4 inch (19mm) - 1 3/8 inches (35mm).
  • Fall Arrest Applications:
    • Maximum 1 PFAS.
    • Structure must withstand loads of at least 5,000 lbs.
    • Maximum Free Fall: 6 feet (or up to 12 feet if used with explicitly certified equipment).

  • Rescue/Confined Space Applications:
    • Structure must withstand loads of at least 3,000 lbs.
    • No free fall permitted.
    • Applicable D-rings: Dorsal, Chest, Shoulder.
  • Connector Compatibility: All connector gates must be self-closing and self-locking, and withstand minimum loads of 3,600 lbs. Incompatible connections, such as two connectors to the same D-ring, irregular applications increasing roll-out risk, or multiple snap hooks/carabiners connected to each other, are prohibited.

Usage Features:

  • Installation Process:
    1. Compress the Anchor Toggle to bring the Spring-Loaded Blade to a parallel position.
    2. Insert the Bolt Hole Anchor through a compatible hole in the steel substrate.
    3. Once the Spring-Loaded Blade has completely cleared the bolt hole, release the Anchor Toggle to allow the blade to return to its perpendicular position.
    4. Verify that the Spring-Loaded Blade has fully returned to its original position; if not, the device MUST NOT be used.
    5. Ensure the Bolt Hole Anchor is fully secured in the installation location.
    6. Connect a complete and compatible PFAS to the Bolt Hole Anchor Connection Point.
  • Pre-Use Planning: Before installation, a Competent Person must ensure all PFAS equipment is compatible, all swing fall risks are eliminated or minimized, and the structure is free of hazards and capable of withstanding the specified minimum loads.
  • Fall Clearance Calculation: Users must account for a minimum 2-foot safety factor, deceleration distance, user height, lanyard/SRL length, harness stretch, and other applicable factors to ensure sufficient clearance below the anchorage connector to arrest a fall before striking the ground or an obstruction.
  • Swing Fall Prevention: Prior to installation or use, considerations must be made to eliminate or minimize swing fall hazards, which occur when the anchor is not directly above the fall location. Users should work as close to in-line with the anchor point as possible.
  • Worker Classifications: The manual defines Qualified Person, Competent Person, and Authorized Person, emphasizing that a Qualified or Competent Person is responsible for supervising the job site and ensuring compliance with safety regulations.
  • Safety Information: The manual stresses the importance of understanding and complying with safety regulations, not altering or misusing equipment, assessing workplace conditions, selecting appropriate fall protection systems under Competent Person supervision, and having a pre-planned rescue procedure. It also warns against using fall protection equipment for lifting tools or equipment unless explicitly certified, and advises consulting a doctor if health conditions may affect a user's ability to withstand fall arrest forces. Trauma relief straps are recommended to reduce the effects of suspension trauma.

Maintenance Features:

  • Cleaning: After each use, the Bolt Hole Anchor must be cleaned to remove dirt, corrosives, and contaminants. Use plain water, or mild soap and water, then rinse and wipe dry. Corrosive substances must NEVER be used for cleaning.
  • Storage: When not in use, the equipment should be stored in an environment protected from heat, light, excessive moisture, chemicals, or other degrading elements.
  • Repair/Serviceability: If the Bolt Hole Anchor fails inspection, it must be immediately removed from service, and Guardian should be contacted for return or repair. Field serviceability testing is not required and should not be performed by the end user.

Inspection Features:

  • Pre-Use Inspection: The user MUST inspect the Bolt Hole Anchor prior to EACH use for deficiencies such as corrosion, deformation, pits, burrs, rough surfaces, sharp edges, cracking, rust, paint buildup, excessive heating, alteration, or missing/illegible labels. Any defects or damage, or exposure to fall arrest forces, requires immediate removal from service.
  • Work Area Inspection: Before each use, ensure the work area is free of damage (debris, rot, rust, decay, cracking, hazardous materials) and is stable, supporting the application-specific minimum loads.
  • Competent Person Inspection: A Competent Person (other than the user) must formally inspect the Bolt Hole Anchor at least every 12 months. These inspections MUST be recorded in the inspection log within the instruction manual and on the equipment inspection grid label. The Competent Person must sign their initials for the month and year of the inspection.
  • Inspection Log: A dedicated inspection log is provided to record Competent Person inspections. This log must be specific to one Bolt Hole Anchor, and all inspection records must be visible and available to all users.
  • Product Lifetime: The product lifetime is indefinite as long as it passes pre-use and Competent Person inspections.
