The short circuit failure
caused by degradation of
motor insulation, cracking of
aged wires, etc., causes phase
loss on inverter output, motor
cable, and motor. Driving the
inverter in such a condition
burns the motor, and
eventually the inverter fails.
Check there is no phase loss by
inspection.
