Standard Limited Warranty

To cancel your account and obtain a refund:
1) Call Lively Customer Service at 1-800-733-6632.
2) You will receive a return authorization number, address, and instructions for return.
3) All equipment must be in “like new” condition, as determined by Lively, in its original box with all
components and materials to be eligible for a refund.
Shipping charges are not refundable. All returns are at the customer’s expense. We will not begin
processing your refund until the device is received by Lively. It can take up to 15business days to process
a refund. Refunds, if applicable, will be processed back to the party that paid in the same manner that
payment was received.
