F Freight carrier account number and special shipping instructions, if necessary
F Item Description / Part Number
F Designated Aircraft Type / Spare
F Contract / Quote Number (if applicable)
F 8130−3 Form Requirements (as necessary)
F End User / User Certification
F Tail Number (if applicable)
