WARRANTY CARD
Terms and conditions of warranty:
1. LIW Care Technology Sp. z o.o., a manufacturer of rehabilitation equipment (ul. Golfowa 7, 94-406 Łódź), guarantees that the device purchased by the
consumer or a different user is in good working order, free from design, assembly and material defects, as well as that the device will remain free from
these defects during the warranty period. The warranty does not cover making the product useful for the purchaser’s needs.
2. LIW Care Technology Sp. z o.o. is granting a two-year warranty for its products. The warranty period runs from the date of issuing the product to the
consumer.
3. An only proof of warranty is this warranty card issued by the seller to the purchaser along with the proof of purchase (invoice or receipt). A transfer of rights
under the warranty requires a transfer of ownership rights to the warranty card and the proof of purchase.
4. If it is found during the warranty period that the equipment is defective or damaged, the defect or damage shall be eliminated free of charge by repairing
or replacing the equipment. If only a part of the product needs to be replaced, the damaged part becomes a property of LIW Care Technology Sp. z o.o. and
shall not be returned to the beneficiary of the warranty.
5. The warranty beneficiary shall report a physical defect in the goods found during the warranty period using a Complaint Form. A file with the Complaint
Form is available at www.liwcare.pl in the „Products” section. This form can also be obtained at each of our Regional Sales Specialists whose contact details
can be found on our website www.licare.pl/kontakt.

6. The warranty beneficiary shall fill out and send the complaint form to the Store where they have purchased the equipment or directly to the Manufacturer
to reklamacje@liwcare.pl.
7. Based on the complaint form received via electronic mail, the Seller decides on how the equipment will be repaired. The Seller can:
a. Order a repair at the warranty beneficiary’s place – in such a case, the equipment repair date is agreed with the beneficiary via e-mail or by
telephone.
b. Require that the equipment be sent to the Manufacturer for repair – in such a case, the warranty beneficiary shall deliver the equipment to
the medical store where they purchased the equipment or to the Manufacturer along with the proof or purchase and a filled-out complaint
form. The defective goods are sent to the Seller after the date of goods collection by a courier has been agreed with the Seller. The delivery
costs are borne by the Manufacturer.
8. The warranty does not cover:
a. worn elements or parts that were destroyed or damaged as a result of misuse (especially including but not limited to the use violating the
instructions for use or the use in inappropriate conditions) or incorrect storage;
b. damage caused by alterations, regulations, adjustments and design changes made by the product user or a third party;
c. damage caused incorrect cleaning or maintenance performed by the product user or a third party;
d. damage caused by normal wear and tear or normal aging of the product;
e. damage caused by negligence on part of the product user (especially including, but not limited to negligence in product maintenance and
cleaning);
f. damage associated with external factors (mechanical damage, contamination, including flooding).
9. The warranty and its terms and conditions are effective within the Republic of Poland only.
10. The warranty does not cover regulations and adjustments performed during the warranty period because these are not product defects.
11. A prerequisite for exercising the warranty is delivering the product to the medical store where the equipment was purchased or to the Manufacturer in a
manner that protects it against damage.
12. The warranty repair or replacement shall be performed within 30 days from the date of delivery of the equipment to the Manufacturer.
13. After the warranty repair, the equipment shall be delivered at the Manufacturer's cost to the address indicated by the warranty beneficiary or to the medical
store where it was purchased. If the repaired product is not collected by the warranty beneficiary, they shall bear all the related costs incurred by LIW Care
Technology Sp. z o.o., including the cost of product storage and transport.
14. The granting of product warranty by LIW Care Technology Sp. z o.o. does not affect the purchaser’s rights under the statutory warranty covering the defects
of purchased goods.
