*BVS S5 Start-up & Training Form MUST BE COMPLETED AND RETURNED
TO BTECH TO INITIATE THE PRODUCT WARRANTY FOR THIS SYSTEM.
This record verifies that the system has been started up propertly. This form should
be completed and returned for each S5 system that is installed. A blank copy of this
form can be found in this Appendix and in the Appendix of each S5’s Installation
Manual.
