15. RETURN OF EQUIPMENT
All request for repairs and replacement parts should be directed to the Customer Service
Department at Misonix, following the outlined procedure below.
1. Call Customer Service (1-800-645-9846) to obtain a Return Material Authorization
Number.
2. Be prepared to provide the serial numbers of all applicable parts.
3. When sending in equipment, always include the generator, convertor and all horns
and probes. Equipment should always be sent in the original packaging.
CAUTION!!
When original packaging is not available, be sure to wrap the convertor and generator
separately in plastic bags, film, or other protective wrapping when using loose packing
materials such as foam pellets, shredded paper, or excelsior. Remove MICROTIP probes or
extenders and pack separately in same carton.

BY RETURNING ANY MATERIAL TO MISONIX INC., THE CUSTOMER OR THE
CUSTOMER'S AGENT THUS CERTIFIES THAT ANY AND ALL MATERIALS SO
RETURNED ARE, OR HAVE BEEN RENDERED, FREE OF ANY HAZARDOUS OR
NOXIOUS MATTER OR RADIOACTIVE CONTAMINATION AND ARE SAFE FOR
HANDLING UNDER NORMAL REPAIR SHOP CONDITIONS. DO NOT RETURN ANY
MATERIAL FOR WHICH SUCH CERTIFICATION CAN NOT BE MADE WITHOUT PRIOR
APPROVAL FROM MISONIX, INC.
