PolyLoop Ligating Device
(
HX-400U-30
)
Quick Reference Guide
Once in place, pull the yellow cylinder
proximally until it stops to extend the
PolyLoop from the plastic sheath.
Remove fingers from the yellow cylinder.
Position the PolyLoop over the polyp at the base
of the stalk, approximately 1-2 mm above the
mucosa. Pull the slider towards the thumb ring
to ligate the tissue. Look at the screen and feel
the resistance in your hand to ensure you do not
transect the polyp stalk. Caution: Do not apply
unnecessary force to the loop when ligating
tissue during the procedure.
With the yellow cylinder still touching the
handle, push the slider to release the
PolyLoop.
If the PolyLoop gets stuck in the tube/coil sheath,
use the Loop Cutter FS-5L/Q/U-1 to cut the
PolyLoop and to detach it from the device.
Refer to instructions on the reverse side for more details.

Summary

Preparation and Instructions for PolyLoop Ligating Device

Preparation Before Use

Remove plastic retainer and extend plastic sheath until PolyLoop is covered. Insert through endoscope working channel.

Extend PolyLoop

Pull yellow cylinder proximally until it stops to extend the PolyLoop from the plastic sheath.

Position and Ligate Polyp

Position PolyLoop 1-2mm above polyp base, pull slider to ligate tissue. Monitor resistance, avoid transecting stalk.

Release PolyLoop

With yellow cylinder touching handle, push slider to release the PolyLoop.

Using and Releasing PolyLoop Ligating Device

Using an Electrosurgical Snare

Look at polyp for ligation signs (blanching/engorging). Close snare 2mm above PolyLoop on stalk.

Releasing Trapped PolyLoop

Straighten endoscope/PolyLoop. Push slider to extend hook, cut sheath/wire, then cut PolyLoop with Loop Cutter.

Overview

The Olympus PolyLoop Ligating Device (HX-400U-30) is a medical device designed to prevent or control bleeding following polypectomy of pedunculated polyps. It delivers a nylon loop snare for ligation.

Function Description:

The primary function of the PolyLoop Ligating Device is to ligate the base of a pedunculated polyp after its removal, thereby preventing or controlling post-polypectomy bleeding. The device utilizes a nylon loop snare that is deployed and tightened around the polyp stalk to achieve hemostasis.

Usage Features:

Preparation:

  1. Removal of Retainer and Sheath Extension: Before use, the plastic retainer must be removed from the PolyLoop. The plastic sheath should then be extended until the PolyLoop is fully covered.
  2. Insertion into Endoscope: The PolyLoop is designed to be inserted through the working channel of an endoscope.

Ligating Procedure:

  1. PolyLoop Extension: Once the device is in place within the endoscope, the yellow cylinder is pulled proximally until it stops. This action extends the PolyLoop from the plastic sheath. After extension, fingers should be removed from the yellow cylinder.
  2. Positioning: The PolyLoop is positioned over the polyp at the base of its stalk, specifically 1-2 mm above the mucosa. This precise positioning is crucial for effective ligation.
  3. Ligation: To ligate the tissue, the slider is pulled towards the thumb ring. During this step, the user should observe the screen and feel for resistance in their hand. This feedback helps ensure proper ligation without transecting the polyp stalk. A critical caution is to avoid applying unnecessary force to the loop when ligating tissue, as this could lead to complications.
  4. Release: After ligation, with the yellow cylinder still touching the handle, the slider is pushed to release the PolyLoop from the device.

Troubleshooting and Maintenance Features:

Releasing a Trapped PolyLoop Ligating Device:

In situations where the PolyLoop becomes trapped, specific steps are outlined to safely release it:

  1. Straightening Endoscope and PolyLoop: The first step is to straighten the endoscope and the PolyLoop as much as possible. This minimizes tension and facilitates subsequent steps.
  2. Extending Hook: The slider is pushed distally to extend the hook from the coil sheath.
  3. Cutting Sheath, Coil, and Operating Wire: Using pliers or a wire cutter, the sheath, coil, and operating wire are cut near the yellow cylinder. This severs the connection to the main device.
  4. Withdrawing Endoscope: After cutting, the endoscope can be withdrawn.
  5. Inserting Scope and Cutting PolyLoop: The scope is then re-inserted next to the sheath. A Loop Cutter (specifically the FS-5L/Q/U-1) is used to cut the PolyLoop between the stopper and the coil sheath. This detaches the PolyLoop from the device.
  6. Removing Sheath: Finally, the sheath is removed from the patient.

Important Note on Trapped Device:

If the PolyLoop gets stuck in the tube/coil sheath, the Loop Cutter FS-5L/Q/U-1 is specifically recommended to cut the PolyLoop and detach it from the device. Further details on this procedure are available on the reverse side of the manual.
