There could be four Possibilities:
The rear camera cable is connected to the sticky mount
power port.
The rear camera's 2.5mm jack is not connected properly.
The rear camera is not securely connected into the
front/main camera's rear cam's port.
You may have disabled the rear camera from the Video
Resolution menu.

Rear camera must be connected to the rearcam/data/power
port located at the top of the front main camera.
Please remove the 2.5mm Jack from the rear camera's
female jack and then re-insert while ensuring those
male/female prongs are set in place so you don't see the
metallic 2.5mm pin.
Remove and re-insert the rear cam cable on the front/main
camera unit on top. Push it gently until you hear a clicking
sound to make sure that it is connected securely. (Make
sure only to plug in the rear camera when the main (front)
camera is turned OFF; otherwise, it may damage the rear
camera)
