Double-click the added custom paper size name in the list and rename it.

Enter the width and height in [Paper Size], and then select your printer name in [Printer
Margins].
 For Mac OS X 10.6.x to 10.7.x, enter the width and height in [Paper Size], and then select your printer name in
[Non-Printable Area].
 Select the printer name in [Printer Margins] to use the automatically entered margin settings.
 Click [-] to delete a custom paper size.

Click [OK].
The Custom Paper Size is saved and you can select it in [Paper Size] from the [Page Attributes] menu.
