Please reference the Extrel MAX300-_RTG Installation Manual for specific information
regarding customer responsibility during installation / startup.
This form must be signed by the responsible individual, verifying that all necessary
preparations have been made. If the Technical Support Representative arrives on-site and
preparations are either incomplete or improper, the commissioning may be terminated. The
customer will then be responsible for all additional cost necessary to complete the
commissioning, and billed at current published rates.
Upon completion, please email support@extrel.com at least fourteen (14) days prior to
scheduling. If possible, please include a map and / or directions for locating your facility, and
information on available lodging.
