Cable not being gripped. Damage to mains plug.

In addition to the above, the plug cover (if it is not moulded) needs to be taken
off and check that the cable terminations are correct.
Examples of faults:
Fuse not being used. Bare wires visible.
EARTH WIRE: Green/Yellow
LIVE WIRE: Brown
NEUTRAL WIRE: Blue
