erence in the future. The space below provides a convenient means of recording the preventive maintenance pro-
cedure described in the Preventive Maintenance section of this manual. PLEASE MAKE COPIES OF THIS ORIG-
INAL FOR YOUR USE.

INSTRUCTIONS: Fill in the equipment serial number and machine number. Fill in the date on which the mainte-
nance procedure was performed. Enter the maintenance procedure code number found in the upper right corner
of the particular maintenance procedure, Example: IX2 and enter the name of the person who performed the main-
tenance procedure. Make copies of this original for your use.
