• Sample chart recorder strips documenting the problem, if applicable.
• Purchase Order to allow tracking of loan equipment.
• Purchase Order for a unit with an expired warranty.
If the unit needs to be sent to ZOLL Medical Corporation, obtain a service order request number from the Technical Service representative. Return
