Aer the device has been implanted and the pocket is closed, interrogate the device and set the Lead Type to
the correct seng. Lead Type sengs are described on the programmer's onscreen help. For MR Condional
pulse generators, set the Addional Hardware parameter to the correct seng.
NOTE:
In CRTPs, the right and le ventricular pulse amplitudes and pulse widths are independently
programmable. The pulse amplitude and pulse width should be evaluated in each chamber accordingly.
In CRTPs, independent lead impedance values are displayed for the RV and LV leads.
Managing and Following Paents
Paent Registraon Form
Fill out and return the Paent Registraon Form to register the paent and facilitate paent tracking.
Paent Idencaon Card
Fill out the Paent Idencaon Card as provided in the outer box with the paent’s name, implant date, and
your name or healthcare facility informaon along with the contact phone number. Record device informaon
(model number, serial number, and UDI number) and leads informaon (model number, serial number, and
UDI number), or apply the labels provided with the sterile package in the designated secons of the card. Fill
out the locaon of the device in the corresponding implant locaon eld on the Paent Idencaon Card.
Fill out the model/length and implant locaon of the leads in the corresponding elds on the Paent
Idencaon Card.
Paent FollowUp
Give the completed card to the paent. To obtain a replacement card if a paent loses or damages their card,
contact
