Warranty and Service Instructions
All requests for return of ACCU‑CHEK Aviva Expert meters under the above warranty or service policy must
be made to ACCU-CHEK Customer Care Service Center. You will be mailed a return authorization label, which
must be afxed to your carton for shipping the system to Roche Diagnostics. Cartons received without this
label will be returned to you at your expense.
Customers experiencing difculties should review the troubleshooting information in Chapter 8 of this booklet.

Be sure to ll out and mail the Warranty Card that comes with the ACCU‑CHEK Aviva Expert system.
