VERIFICATION TESTING & A FORM 2 METER
Step 3.1—Select NO if this is not the correct meter type & rotation.
Step 3.2—Select the correct meter type & rotation. For Forms 1 or 2 select ABC.

METER TYPE for a Polyphase Meter METER TYPE for a Form 1, 2, or 12 Meter
Step 3.2—If you are incorrect as to the rotation, you will see the following message. Press
