F.3.1.3 Form 5 Meters—Form 5 meters are polyphase meters that have 2 current
coils and 2 stators. They are a CT rated meter. It will require 2 tests to determine the
accuracy of the meter. The automatic phase rotation test is now important in determining
the meter accuracy. The hookup is as follows for the 4 different configurations of a form 5
meter:

Automatic Phase Rotation Test—Voltage Cable Hookup:
o Red alligator clip—Phase A
o White alligator clip—Neutral
o Black alligator clip—Phase B on the lug above the switch & leave
there for the entire test
Test 1: Test Phase A
Short out CT on phase B
Secondary Voltage Cable
o Red alligator clip to phase A
o White alligator clip to neutral. This produces V

o Black alligator clip to phase B on the lug above the switch & leave
there for the entire test
Secondary Current Cable—Duckbill connector copper side up phase A (I

Test 2: Test Phase B
Short out CT on phase A
Secondary Voltage Cable
o Red alligator clip to phase B
o White alligator clip to neutral. This produces V
