If requested by the telephone company, the following information must be
provided before connecting the CCDACT to telephone lines:
• Original manufacturer: Simplex Time Recorder Company
• Model Number: 4004-9810, 4005-9810, 4020-0153, 4100-0153, or
4120-0153
• FCC Registration Number: 5QWUSA-23579-AL-E
• Type of jack(s) (to be installed by the telephone company): RJ-31X
Important: Do not direct-connect the CCDACT to a coin telephone or party
line.
If the CCDACT harms the telephone network, the telephone company will notify
you in advance that temporary discontinuance of service may be required. If
advance notice is not practical, the telephone company will notify you as soon as
possible. You have the right to file a complaint with the Federal
Communications Commission should you believe it necessary.
The telephone company may make changes in its facilities, equipment,
operations, or procedures that could affect the operation of the CCDACT. If this
happens, the telephone company will provide advance notice so that you can
make the necessary modifications to maintain uninterrupted service.
