Note: If your 4004 panel has firmware that is at Revision 1.03.01, read the
“4004 Installations” section (on page 2-2) before starting the procedure
below.

1. (If applicable) Disconnect one of the two battery terminals. Then remove
AC power at the power source.
2. (If applicable) Disconnect and remove the optional City Card(s). Tie off
and remove the City Circuit wiring.
3. Remove the screws that secure the CPU board chassis to the back box.
4. Remove the chassis from the back box.
5. Screw the four standoffs (provided) to the rear of the CPU board chassis.
Holes for the standoffs are pre-drilled and tapped.
6. Using the screws provided, secure the CCDACT to the standoffs. Ensure
that mounting lug near TB4 is snug (the lug provides ground for electrical
transients).
7. Loop the tie-wrap (supplied) around the stand-off near TB4; leave enough
play in the loop to let at least 2 fingers pass through. This loop will provide
strain relief for the telephone company (TELCO) cables.
8. Slide the CPU board chassis into place and secure the chassis with its four
screws.
9. Affix the telephone line registration label (supplied) to the FACP enclosure
at the location shown.
