8.3 Returning the light source for repair

When returning the light source for repair, contact Olympus. With the light source, include a description
of the malfunction or damage and the name and telephone number of the individual at your location
who is the most familiar with the problem. Also, include a repair purchase order.
CAUTION
Olympus is not liable for any injury or damage that occurs as a result of repairs
attempted by non-Olympus personnel.
NOTE
If an accessory of the light source (e.g., examination lamp, foot holder, power cord,
light source cable etc.) needs to be replaced, contact Olympus to purchase a
replacement.
