Dead on Arrival
Criteria and Procedure
To improve quality especially of the RATIONAL production and assembly line any failure or breakdown directly
aer delivery has to be reported via the “Dead on Arrival (DoA)” procedure, aer xing the problem.
Criteria
1. The time between unit installation and unit failure is less than 7 days; except hidden failures which were
present but not discovered during installation, e.g. insucient welding of the interior cabinet.
2. The unit was installed not later than one year aer production date.
3. Complete data and replaced parts are available for investigation.
4. The unit was not altered before installation and not used as a training or test unit.
5. The failure is not caused due to transportation, improper installation or wrong handling.
6. Service parts or additional ordered accessories will not be evaluated (exception: UltraVent)
Reporting Procedure
1. In case service data are available, they have to be downloaded. (service data, HACCP data and log les)
2. The Partner will document the unit installation and the surrounding conditions with photos and installation
checklist.
3. If applicable, exchanged parts have to be sent immediately to RATIONAL in Landsberg (address as indicated
on the DoA registration form) to be available for investigation. All replaced parts need to be sent back to
RATIONAL.
4. RATIONAL UPS account (74843W) can be used for shipment.
5. A copy of the DoA registration form has to be attached to the part shipment plus commercial invoice with
the residual value (see online).
6. The DoA registration form (see attached template) has to be lled in either in English or German and send
to the indicated address on the form (either lled digital or in a readable written form) including tracking ID.
7. Additional data like pictures should be uploaded to RATIONAL by using the following link:
doa.rational-online.com.
8. RATIONAL will inform the Partner about results and actions taken.
In the instance that a Product Survellance or Dead on Arrival needs to be submitted.
