ACC U -C H EK Active L i m ited 3-Year W arranty
R oche D iagnostics warrants to the original purchaser of the m eter that your ACC U -C H EK Act ive m eter will be free fro m defects in m aterials and
work m anship for 3 years fro m the date of purchase. If, during t his 3-year period, the m eter does not work properly bec ause of a defect in m aterials or
work m anship, R oche D iagnostics will replace it with a new ACC U -C H EK Act ive m eter or equivalent product free of charge. The warranty of the
replace m ent m eter will expire on the date of the original warranty expiration or ninety ( 90 ) days af ter the ship m ent of a replace m ent syste m , whichever
period is longer. The purchaser’s exclusive re m edy with respect to the ACC U -C H EK Act ive m eter shall be replace m ent.
This warranty does not apply to the performance of an ACCU-CHEK Active meter that has been damaged by accident or has been altered ,
misused , tampered with , or abused in any way . Roche Diagnostics will handle meters that show damage or abuse according to its Non-Warranty
Service Policy described below .
THE ABOVE WARRANTY IS EXCLUSIVE OF ALL OTHER WARRANTIES , AND ROCHE DIAGNOSTICS MAKES NO OTHER WARRANTIES , EXPRESS OR
IMPLIED , INCLUDING WITHOUT LIMITATION , THE IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE . IN NO
EVENT SHALL ROCHE DIAGNOSTICS BE LIABLE TO THE PURCHASER OR ANY OTHER PERSON FOR ANY INCIDENTAL , CONSEQUENTIAL , INDIRECT ,
SPECIAL , OR PUNITIVE DAMAGES ARISING FROM OR IN ANY WAY CONNECTED WITH THE PURCHASE OR OPERATION OF THE METER OR ITS PARTS .
NO WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE , IF ANY IS IMPLIED FROM THE SALE OF THE METER , SHALL
EXTEND FOR A LONGER DURATION THAN THREE YEARS FROM THE DATE OF PURCHASE .

eli m ination and exclusion m ay not apply to you. This warranty gives you specific legal rights, which vary fro m state to state.

N on- W arranty S ervice Policy
R oche D iagnostics’ N on- W arranty S ervice Policy applies to m eters where the above warranty has not beco m e effective, has bec o m e inappli cable, or has
expired. R oche D iagnostics will replace at its option m eters returned to it for a service charge ( not to exceed $35 ) . R eplace m ent will be with the sa m e or
si m ilar product. R eplace m ent m eters will be warranted for a period of 90 days fro m ship m ent under a li m ited warranty providing for replace m ent of parts
and labor at no charge.
