1. Installation report/warranty passport:
• The serial numbers from the
– dentist element,
– assistant element,
– water unit,
– chair,
–lamp,
–monitor
– SIROCAM 3 / SIROCAM C,
must be entered in the "Installation report/warranty
passport" document.
• Enter the equipment of the dentist element.
• Complete the installation report
• Complete the remaining lines with the customer.
• Hand over the CUSTOMER'S COPY to the customer.
The DEALER'S COPY is for the customer records kept
by the dealer.
2. Dealer:
Please fax the DEALER'S COPY to Sirona directly af-
ter the installation.
ATTENTION
Please note that any warranty claims can be fulfilled
only if the completely and legibly completed "Installa-
tion report/warranty passport" document is available at
Sirona directly after the installation!
3. For the Customer …
Accessories (depending on order):
The accessories shown above for sanitization, water
unit, assistant element and for the thermo-disinfectable
supply hoses must be handed over to the customer.
