To order additional copies of this document, send or call in an order as follows:
Lucent Technologies entities should use Form IND 1-80.80 FA, available through
the Customer Information Center.
One-time orders include a binder (if applicable) and the document contents for the
current issue in effect at the time of order. After placing a one-time order, you may
request placement on the standing order list for all later reissues of the document.
The standing order list for each document provides automatic distribution for all
reissues of the document. Normally, these reissues contain only the unbound
document packaged in shrink-wrap material for shipment.
