5.5 Push (not in High Flow mode)
In addion to the basic CPAP mode, the medin-NC3® can increase the CPAP pressure if necessary for a short me.
These periods are called "pushes" and can be used to smulate the paent. A push is determined by the addional
ow used for it and its duraon.
Addional ow during a push - F
insp
:
In eld c in Figure 5-2, set an addional ow (F
insp
) by pressing the buon to acvate the seng and turning the
dial to select a value which immediately becomes acve for the next push and then pressing the dial to conrm. If
the dial is not pushed, the change is made nonetheless. During a push, the medin-NC3® emits, as a ow, the sum
of the set basic ow (Flow) in Figure 5-2 eld a and the addional push ows (F
insp
).
As an addional push ow F
insp
a value between 0 L/min (that is, essenally no push is triggered) and a maximum
value dependent on the basic ow set can be set. The maximum value is 17.5 L/min minus the set basic ow Flow.
Warning:
A push does not generate a breath; instead it can only smulate but not venlate!
The seng F
insp
may only be changed in small increments. The eect of each increment (= CPAP pressure that
develops during a push) must be monitored by triggering a manual push or by waing for an automac push. It
should be noted that any change becomes immediately acve for the next push, even without pressing the dial.
The CPAP pressure that develops during a push depends on the leakage of the CPAP system (parcularly the
leakage from prongs or the mask). An increase in the push ow by +1 L/min can cause an increase of up to 3.5
mbar in the CPAP pressure.
Since leakage from the mask and prongs causes a decrease in the pressure during a push, this leakage must be
checked before increasing the push ow. An increase in the push ow is not a substute for leak-proof posioning
of the mask or prong!
During a push, the set basic ow (Flow) and addionally the set push ow (F
insp
) are emied such that an increase
in the basic ow also causes an increase in the CPAP pressure during a push.
When seng the addional ow during a push, the following must be ensured:
The alarm CPAP pressure high briey stops the ow to the paent and sounds an alarm starng at a
CPAP pressure of 18 mbar; that is, if the push is set such that its pressure is very high, a slight change
in the leakage or the expiraon against push by the paent will cause the medin-NC3® to briey stop
the ow and trigger an alarm. To prevent this, the push ow must be reduced.
If a very high overall ow is set in the event of a push, high stac pressure may develop in the tube
system. If this exceeds 60 mbar, the Feed pressure high alarm is triggered and the ow emission to
the paent is stopped. In this case, the push ow set must be reduced and the leakage at the paent
reduced. Depending on the duraon of the high supply pressure, the medin-NC3® automacally
restarts the ow aer a short me or not unl aer conrmaon from the user (if Press alarm reset
to restart ow is displayed).
During a push, if a ow is set that is so high that it triggers the Feed pressure high alarm and
simultaneously a CPAP pressure high alarm, it is possible that the text Press alarm reset to restart
ow may also be displayed. In this case, the medin-NC3® stops the ow unl the user restarts it by
pressing the Alarm Reset buon.
