e Manufacturer, its distributors or suppliers shall not be respon-
sible for any incidental, special or consequential damages of any kind
or nature caused by or arising out of a defect or malfunction of the
Pump.
All recommendations, information, and descriptive literature supplied
by the Manufacturer or its agents are believed to be accurate and reli-
able, but do not constitute warranties.
Warranty Procedure
Notice of the claimed warranty defect must be made in writing, fax
or by telephone to the Manufacturer as follows: Smiths Medical MD,
Inc., 1265 Grey Fox Road, St. Paul, MN 55112 U.S.A. Telephone:
1 800.426.2448, or if outside the USA contact your local distributor.
Original Purchaser must include date of purchase by the Original
Purchaser, model and serial number, and a description of the claimed
warranty defect in sucient detail to allow the Manufacturer to deter-
mine and facilitate any repairs which may be necessary.
AUTHORIZATION MUST BE OBTAINED PRIOR TO RETURN
ING THE PUMP. If authorized, the Pump must be properly and care-
fully packaged and returned to the Manufacturer, postage prepaid. Any
loss or damage during shipment is at the risk of the sender.
