Problem/Error Message Possible Cause Suggested Solution
First Test BI positive
result and second or third
test BI positive result.
Fluorescent
contamination on
BI, sterilization not
achieved, improperly
placed BI, ingress
window on the
BI cap blocked.
May be due to
overloaded chamber,
wet load, material
incompatibility,
BI misuse, or
defective BI.
Follow current hospital or
healthcare facility policy
and procedures regarding
quarantine, or retrieval, and
reprocessing of potentially
non-sterile instruments and
notication of the clinician(s).
Report the positive BI to
ASP including the product
lot number.
Have the sterilizer checked
by ASP and do not use the
sterilizer until unit has
been checked.
BI ampoule breaks during
sterilization.
Ampoule defect. Reprocess load with new BI.
Chemical indicator
indicates exposure prior
to sterilization cycle.
Improper storage or
handling.
Follow hospital or healthcare
facility policy and procedures
on disposal of the BI. Process
the load with a new BI.
Reader touch screen does
not illuminate.
Power failure,
backlight failure.
Check if the mains power is
working. Check if the unit is
plugged in. Unplug the power
adapter from the back of
the reader. Wait 30 seconds.
Reinstall the power adapter in
the back of the reader. If the
problem persists, contact ASP.
Excessive amount of
liquid spilled on reader.
Accidental spill, roof
leak, burst pipe.
Unplug the reader being
careful not to touch any of the
spilled liquid. Do not attempt
to dry internal components.
Contact ASP.
