A form is a data collection template with input prompts. Data will be kept as a
transaction record after the user completes all the input fields (lines) of a form. Then, it
can be saved in a transaction file, added to its lookup database, or directly output to a
printer, etc.
 Each form allows at most eight input fields (8 lines).
 Ten different forms can be defined.
