Returning Circuit Boards
Package circuit boards for shipment as follows:
1. Place each circuit board in an electrostatic discharge (ESD) bag or container.
2. Provide a separate packing container for each circuit board.
3. Attach a tag to the container that includes the Return Authorization Number
and the information (hospital, phone number, etc.) listed in Step 1-Obtain a
Return Authorization Number.
4. Ship the circuit board prepaid to the Valleylab Service Center.
