Appendix G: Warranty
Limited Four-Year Warranty
Roche Insulin Delivery Systems Inc. warrants the ACCU-CHEK
®
Spirit Combo insulin pump (“insulin pump”) against defects in
materials and workmanship for a period of four (4) years from
the date of purchase. Proof of purchase date may be required.
This warranty does not include supplies and accessories, including
but not limited to, cartridges, batteries, or infusion sets.
During the warranty period, Roche Insulin Delivery Systems Inc.
will replace, in its sole discretion, any insulin pump that does not
work properly because of a defect in materials or workmanship.
The purchaser’s sole and exclusive remedy with respect to the
insulin pump shall be replacement. This warranty applies only to
new devices. In the event an insulin pump is replaced, the
warranty period will not be extended.
