AV Sequential or DDD Pacemakers
For pacemaker patients, follow these steps:
z Always attach at least four electrodes to allow for a choice of leads.
z Turn pacer detection on.
z Select the lead with the highest R-wave and the least interference.
z Verify that the HR calculation is accurate by comparing it to the ECG
waveform.
z Monitor the pulse or the respiration or both using a method other than ECG.
For patients without pacemakers, turn the pacer detection off.
WARNING: Fusion mode pacer detection is not intended for
use with large-voltage, unipolar pacemakers. It is intended
for use only with biphasic pacemakers. Please observe the
following:
z Before selecting Pacer Fusion Mode, be certain that the
patient has a biphasic pacemaker (external or
implanted) and that it is accurately programmed as
appropriate for that patient.
z Do not select Fusion Mode if you are uncertain what
type of pacemaker is in use or how it is programmed.
z Select Fusion Mode only to suppress repeated false
asystole and/or false low heart rate alarms.
z Selecting Pacer Fusion Mode may increase the risk of
the monitor falsely counting pacer spikes as QRS
complexes and may cause cardiac arrest to go
undetected. Therefore, special surveillance of any
pacemaker patient monitored with this mode is strongly
recommended.
WARNING: The monitor may not count heart rates accurately
and may misinterpret rate-dependent arrhythmia in some
paced patients. Do not rely entirely on the displayed heart
rate to assess a paced patient’s condition. Always observe
these patients closely and monitor all of their vital signs
carefully.
