2. To alter any information entered in Incident Setup, click the
button. A message box will appear asking if you are sure you want to change the Incident
Setup Values. If Yes is selected, the disabled (grayed-out) boxes will become enabled.
Note: Only change the Incident/Site ID if there is something unique about the incident or
site for this patient.

3. Patient Information.
Patient ID: A unique ID assigned to each patient (ex. social security number). This
field is required and can be any combination of alphanumeric characters to a
maximum of 20 characters.
Sex: Select male or female from the drop-down list box or press the M or F key. This
field is required.
