applicable to the particular installation, the “Comments” field must be marked “N/A”. Dates should be in the dd-mmm-yyyy format (day-month-year, e.g.
12-Nov-2006).
A satisfactory operational status means that all major operating facilities and requirements of the equipment or unit have been tested and found to
function in accordance with the relevant SOLAS regulations and associated IMO performance standards.
