Tax Authorization Form 8821
These forms provide authorization from the borrower to request tax information or copies
of tax returns from the IRS.
Tax Return Request–Copy of Return (Form 4506–T)
This form is the borrowers’ authorization to order transcripts from the Internal Revenue
Service of the following tax forms:
• Tax return
• Tax account information
